Requests are intended for Dr Hani Samai and his team to arrange a callback and prepare a possible appointment.
Name, phone number, city, contact preferences and practical form answers. We do not request medical records, clinical images, identification documents or payment information.
Information is stored in a private WordPress area accessible to authorized administrators. Visit preferences may help prioritize callbacks; they do not decide access to care. If the owner configures Google Sheets, a limited export of contact and follow-up details may be sent to their private spreadsheet.
This system does not send form contact details, answers or scores to advertising tools. Measurement tags may be enabled on the presentation page where platform rules permit, only after your choice. Refusing does not prevent you from sending a request. Cookie preferences remain available while measurement is active.
Information is used to process and follow up your request. You can contact the team during your callback to request access, correction or deletion. Do not submit medical information through this form.
